Summary

Antacids may modestly reduce digoxin absorption when taken together. The effect is usually small, though reduced digoxin levels could matter in patients with a narrow margin of control. Hypercalcemia from high calcium intake can also increase digoxin toxicity.

What this interaction means in plain words

Calcium carbonate and Digoxin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Calcium carbonate belongs to Gastrointestinal Tract; Digoxin belongs to Cardiovascular.

Evidence level: Low

Why it happens

Binding or adsorption of digoxin in the gut reduces absorption, and hypercalcemia sensitizes the myocardium to digoxin.

What to do

Separate digoxin and calcium carbonate by at least 2 hours. Monitor digoxin levels, serum calcium and heart rate or rhythm if high doses of calcium are used.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
