Summary

Posaconazole raises exposure to cariprazine and its active metabolites, increasing the risk of akathisia, extrapyramidal symptoms and sedation. The long half-life of cariprazine's active metabolites means effects persist after the inhibitor is stopped.

What this interaction means in plain words

Cariprazine and Posaconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Cariprazine belongs to Central Nervous System; Posaconazole belongs to Anti Fungals.

Evidence level: High

Why it happens

Posaconazole inhibits CYP3A4, which is the main metabolic pathway of cariprazine.

What to do

Reduce the cariprazine dose according to labeling when a strong CYP3A4 inhibitor is started, and monitor for extrapyramidal symptoms. Adjust the dose again when posaconazole is withdrawn.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
