For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Additive CNS depression and psychomotor impairment; may increase dizziness, drowsiness, confusion, and risk of falls. Potential increase in anticholinergic burden (dry mouth, constipation, urinary retention), particularly in older adults.

Mechanism of action

Pharmacodynamic: combined sedative effects from H1-antagonism (cetirizine) and TCA sedating/anticholinergic properties (clomipramine). No meaningful pharmacokinetic interaction expected.

Clinical management

Use the lowest effective doses; avoid or limit alcohol and other CNS depressants; counsel to avoid driving or hazardous tasks until individual response is known; monitor for excessive sedation, confusion, and falls (especially elderly); consider a less sedating antihistamine (e.g., fexofenadine) if sedation occurs.
