Summary
Opioids combined with sedating antipsychotics can cause profound sedation, respiratory depression, hypotension, coma and death. Chlorpromazine adds CNS depressant and alpha-blocking hypotensive effects to those of oliceridine.
What this interaction means in plain words
Chlorpromazine and Oliceridine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Chlorpromazine belongs to Central Nervous System; Oliceridine belongs to Musculoskeletal.
Why it happens
Additive CNS and respiratory depression from mu-opioid agonism (oliceridine) and chlorpromazine's sedative effects, with additive hypotension.
What to do
Reserve the combination for patients in whom other options are inadequate, using the lowest doses and shortest duration. Monitor closely for sedation, respiratory depression and hypotension.
