For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Clarithromycin can substantially increase pacritinib exposure through CYP3A4 inhibition. This raises the risk of pacritinib toxicities such as QT prolongation, bleeding, diarrhea and cytopenias. Pacritinib labeling advises against use with strong CYP3A4 inhibitors.

What this interaction means in plain words

Clarithromycin and Pacritinib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Clarithromycin belongs to Anti Bacterial; Pacritinib belongs to Oncology.

Evidence level: Moderate

Why it happens

Strong CYP3A4 inhibition decreases the metabolic clearance of pacritinib.

What to do

Avoid the combination and select a non-CYP3A4-inhibiting antibiotic such as azithromycin. If it cannot be avoided, monitor ECG and blood counts closely and consider holding pacritinib.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
