Summary

Voriconazole increases pacritinib exposure through CYP3A4 inhibition. Elevated pacritinib levels may increase risks of QT prolongation, bleeding, diarrhea and other dose-related toxicities.

What this interaction means in plain words

Pacritinib and Voriconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Pacritinib belongs to Oncology; Voriconazole belongs to Anti Fungals.

Evidence level: Moderate

Why it happens

Strong CYP3A4 inhibition decreases the metabolic clearance of pacritinib.

What to do

Avoid concomitant use of pacritinib with strong CYP3A4 inhibitors; choose a non-azole antifungal with minimal CYP3A4 inhibition where possible. If unavoidable, follow labeling and monitor ECG closely.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
