Summary
Velpatasvir inhibits intestinal P-glycoprotein and can raise colchicine exposure. Higher colchicine levels increase the risk of serious toxicity, including neuromuscular effects, bone marrow suppression and gastrointestinal toxicity, especially in renal or hepatic impairment.
What this interaction means in plain words
Colchicine and Velpatasvir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Colchicine belongs to Genitourinary; Velpatasvir belongs to Anti Virals.
Why it happens
Inhibition of P-glycoprotein efflux reduces colchicine intestinal secretion and clearance, increasing systemic exposure.
What to do
Reduce the colchicine dose or frequency according to labeling when coadministration is needed, and monitor for colchicine toxicity. Avoid the combination in patients with renal or hepatic impairment.
What to ask your pharmacist
