Intravenous erythromycin inhibits CYP3A4 and can raise dasatinib exposure, increasing the risk of myelosuppression, fluid retention and pleural effusion. Both drugs also prolong the QT interval, adding arrhythmia risk.

What this interaction means in plain words

Dasatinib and Erythromycin lactobionate can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Dasatinib belongs to Oncology; Erythromycin lactobionate belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Erythromycin inhibits CYP3A4-mediated metabolism of dasatinib, and both agents have additive QT-prolonging effects.

What to do

Choose a non-macrolide antibiotic where possible. If unavoidable, monitor ECG, electrolytes and dasatinib toxicity, and consider reducing the dasatinib dose.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
