Summary
Levoketoconazole is a strong CYP3A4 inhibitor and also prolongs the QT interval, and dasatinib is a CYP3A4 substrate that prolongs QT as well. Dasatinib exposure may rise substantially, increasing the risk of toxicity and arrhythmia.
What this interaction means in plain words
Dasatinib and Levoketoconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Dasatinib belongs to Oncology; Levoketoconazole belongs to Endocrine.
Why it happens
Strong CYP3A4 inhibition reduces dasatinib clearance, with additive QT prolongation.
What to do
Avoid the combination. If it cannot be avoided, reduce the dasatinib dose per labeling for strong CYP3A4 inhibitors and monitor the ECG and for dasatinib adverse effects.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
