Summary

Dolasetron's active metabolite hydrodolasetron prolongs the QRS and QT/QTc intervals, and volatile anesthetics such as desflurane can also lengthen repolarization. Together they may increase the risk of torsades de pointes during anesthesia, particularly if electrolytes are disturbed.

What this interaction means in plain words

Desflurane and Dolasetron can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Desflurane belongs to Central Nervous System; Dolasetron belongs to Gastrointestinal Tract.

Evidence level: Low

Why it happens

Additive prolongation of cardiac repolarization (QTc) through pharmacodynamic effects on cardiac ion channels.

What to do

Consider a different antiemetic such as a non-QT-prolonging agent in patients with QT risk factors. Correct potassium and magnesium before surgery and monitor the ECG perioperatively.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
