Summary
Loop diuretics such as ethacrynic acid cause potassium and magnesium depletion, which increases myocardial sensitivity to digoxin. This can precipitate arrhythmias even at normal digoxin levels.
What this interaction means in plain words
Digoxin and Ethacrynic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Digoxin belongs to Cardiovascular; Ethacrynic acid belongs to Cardiovascular.
Why it happens
Diuretic-induced hypokalemia and hypomagnesemia enhance digoxin binding to Na+/K+-ATPase and its proarrhythmic effect.
What to do
Monitor serum potassium and magnesium regularly and replace losses. Consider potassium-sparing therapy and monitor for digoxin toxicity.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
