Summary
Thiazide-induced hypokalemia and hypomagnesemia sensitize the myocardium to digoxin and can precipitate digoxin toxicity and arrhythmias. The ACE inhibitor partly offsets potassium loss but renal function changes can also raise digoxin levels.
What this interaction means in plain words
Digoxin and Hydrochlorothiazide-lisinopril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Digoxin belongs to Cardiovascular; Hydrochlorothiazide-lisinopril belongs to Cardiovascular.
Why it happens
Diuretic-induced hypokalemia and hypomagnesemia increase myocardial sensitivity to digoxin, and changes in renal function may reduce digoxin clearance.
What to do
Monitor serum potassium, magnesium and renal function regularly and keep potassium in the normal range. Check digoxin levels and watch for signs of toxicity.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
