Summary

Disopyramide prolongs the QT interval and can cause torsades de pointes. Fluoxetine may add to this risk. Fluoxetine inhibits CYP2D6 and may also modestly alter disopyramide handling, and disopyramide adds anticholinergic effects.

What this interaction means in plain words

Disopyramide and Fluoxetine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Disopyramide belongs to Cardiovascular; Fluoxetine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive QT prolongation, with possible pharmacokinetic effects of fluoxetine on disopyramide.

What to do

Avoid if possible, and consider an antidepressant with lower QT risk. If used together, monitor ECG and electrolytes and watch for anticholinergic effects and arrhythmia.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
