Disopyramide is a class Ia antiarrhythmic that prolongs the QT interval and can cause torsade de pointes. Vilanterol adds beta-adrenergic cardiac stimulation and may reduce potassium, increasing proarrhythmic risk.
What this interaction means in plain words
Disopyramide and Fluticasone-vilanterol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Disopyramide belongs to Cardiovascular; Fluticasone-vilanterol belongs to Respiratory.
Why it happens
Additive QT prolongation with beta2-agonist-induced hypokalemia and sympathetic stimulation.
What to do
Monitor ECG and serum potassium and magnesium closely, and correct abnormalities. Consider a non-LABA alternative in patients with additional QT risk factors.
