Summary

Disopyramide prolongs the QT interval and can cause torsades de pointes. Sertraline has a smaller QT effect that may add to this, and disopyramide's anticholinergic effects and negative inotropy add further cardiac burden.

What this interaction means in plain words

Disopyramide and Sertraline can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Disopyramide belongs to Cardiovascular; Sertraline belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive QT prolongation.

What to do

Obtain a baseline and follow-up ECG and keep potassium and magnesium normal. Choose another antidepressant or antiarrhythmic if QTc lengthens.
