Summary

Dofetilide prolongs the QT interval and carries a risk of torsades de pointes. Systemic lidocaine has little QT effect and is not a known contraindication, so the risk is mainly the baseline risk of dofetilide. The legacy contraindication is not well supported for lidocaine.

What this interaction means in plain words

Dofetilide and Lidocaine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dofetilide belongs to Cardiovascular; Lidocaine belongs to Cardiovascular.

Evidence level: Low

Why it happens

Possible additive cardiac electrophysiologic effects; lidocaine does not meaningfully prolong QT or inhibit dofetilide elimination.

What to do

Monitor ECG and QTc and keep potassium and magnesium within normal range when systemic lidocaine is used in a patient on dofetilide. Local or dental lidocaine requires no change.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
