Summary
Antipsychotics and triptans have been associated with serotonin syndrome, although the risk appears low and the evidence is largely from case reports. Perphenazine's dopamine blockade could theoretically contribute to neuroleptic malignant syndrome features. Eletriptan is also metabolized by CYP3A4, which perphenazine does not meaningfully inhibit.
What this interaction means in plain words
Eletriptan and Perphenazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eletriptan belongs to Central Nervous System; Perphenazine belongs to Central Nervous System.
Why it happens
Possible additive serotonergic and dopaminergic effects; the mechanism is not firmly established.
What to do
Counsel patients to report agitation, tremor, fever, sweating or muscle rigidity, and monitor for these after starting or increasing either drug. Stop the triptan and evaluate urgently if serotonin toxicity is suspected.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
