Summary

Eliglustat can prolong the PR, QRS and QT intervals at higher exposures, and ondansetron adds QT prolongation. Together they may increase the risk of ventricular arrhythmias.

What this interaction means in plain words

Eliglustat and Ondansetron can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Eliglustat belongs to Miscellaneous; Ondansetron belongs to Gastrointestinal Tract.

Evidence level: Low

Why it happens

Additive QT prolongation; ondansetron is also partly metabolized by CYP2D6 and CYP3A4, which can alter eliglustat-related exposure in some patients.

What to do

Prefer a non-QT-prolonging antiemetic. If combined, obtain a baseline ECG, correct electrolytes and monitor for arrhythmia.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
