Summary

Metoclopramide blocks dopamine D2 receptors and can oppose the dopaminergic therapy used in Parkinson's disease, of which entacapone is an adjunct to levodopa. This may worsen parkinsonian symptoms and reduce the benefit of treatment.

What this interaction means in plain words

Entacapone and Metoclopramide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Entacapone belongs to Central Nervous System; Metoclopramide belongs to Gastrointestinal Tract.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism: central D2 blockade by metoclopramide counteracts the dopaminergic effect enhanced by COMT inhibition.

What to do

Avoid metoclopramide in patients with Parkinson's disease on entacapone/levodopa. Use domperidone or another non-central antiemetic if needed, where available.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
