Summary
Ergotamine is metabolized by CYP3A4 and causes vasoconstriction. Lurasidone does not inhibit CYP3A4, so the main concern is pharmacodynamic: antagonism of dopamine and serotonin signaling and additive alpha-adrenergic effects. Serotonin syndrome is theoretically possible but rarely reported.
What this interaction means in plain words
Ergotamine and Lurasidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ergotamine belongs to Central Nervous System; Lurasidone belongs to Central Nervous System.
Why it happens
Pharmacodynamic overlap at serotonergic and adrenergic receptors; no pharmacokinetic interaction is established.
What to do
Monitor for signs of serotonin toxicity and for blood pressure changes. Avoid concurrent CYP3A4 inhibitors with ergotamine.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
