Summary
Ribociclib inhibits CYP3A4 and may raise ergotamine concentrations. Ergotamine toxicity causes severe peripheral and cerebral vasospasm and ischemia, and ribociclib itself can prolong the QT interval.
What this interaction means in plain words
Ergotamine and Ribociclib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Ergotamine belongs to Central Nervous System; Ribociclib belongs to Oncology.
Why it happens
Ribociclib inhibits CYP3A4, reducing the metabolism of ergotamine.
What to do
Avoid the combination and use a non-ergot acute migraine treatment. If unavoidable, use minimal ergotamine doses and monitor for ischemic symptoms.
