Summary
Intravenous erythromycin prolongs the QT interval, and paliperidone does too, so the combination raises the risk of torsades de pointes. Erythromycin also inhibits P-glycoprotein, which may modestly raise paliperidone exposure.
What this interaction means in plain words
Erythromycin lactobionate and Paliperidone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Erythromycin lactobionate belongs to Anti Bacterial; Paliperidone belongs to Central Nervous System.
Why it happens
Additive QT prolongation, plus P-glycoprotein inhibition that increases paliperidone absorption.
What to do
Use a non-QT-prolonging antibiotic where appropriate. If the combination is necessary, obtain an ECG, correct potassium and magnesium, and monitor the QTc.
What to ask your pharmacist
