Summary
Repotrectinib is mainly cleared by CYP3A4, and strong or moderate CYP3A inducers markedly reduce its exposure. Eslicarbazepine has modest CYP3A4 induction, so lower repotrectinib levels and reduced anticancer activity are possible.
What this interaction means in plain words
Eslicarbazepine and Repotrectinib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Eslicarbazepine belongs to Central Nervous System; Repotrectinib belongs to Oncology.
Why it happens
Eslicarbazepine induces CYP3A4, increasing the metabolism of repotrectinib (the CYP2E1 route cited is not the established mechanism).
What to do
Choose a non-inducing antiseizure drug such as levetiracetam where feasible. If it must be combined, monitor tumor response closely.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
