Esomeprazole + Sulpiride

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Esomeprazole can induce hypomagnesemia with prolonged use; electrolyte disturbances (low Mg/K/Ca) increase the risk of QT prolongation and torsades de pointes with sulpiride, which has QT-prolonging potential. Risk is higher with long-term PPI therapy, high sulpiride doses, renal impairment, or baseline QT risk factors.

Mechanism of action

Pharmacodynamic: PPI-induced hypomagnesemia reduces cardiac repolarization reserve, potentiating sulpiride’s QT-prolonging effects.

Clinical management

Assess and correct Mg, K, and Ca before and during combined therapy (especially if PPI use >2–3 months or if symptoms occur). Consider baseline/periodic ECG in patients with QT risk factors. Use lowest effective sulpiride dose and adjust for renal impairment. Reevaluate PPI necessity or consider alternatives if recurrent or refractory hypomagnesemia.
