Summary
Combining a loop diuretic with a thiazide produces sequential nephron blockade with a markedly enhanced diuresis. This can cause profound loss of potassium, sodium, magnesium and volume, leading to hypokalemia, hyponatremia and hypotension.
What this interaction means in plain words
Ethacrynic acid and Hydrochlorothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ethacrynic acid belongs to Cardiovascular; Hydrochlorothiazide belongs to Cardiovascular.
Why it happens
Blockade of sodium reabsorption at both the loop of Henle and distal tubule produces additive natriuresis and potassium wasting.
What to do
Monitor electrolytes, renal function, weight and blood pressure closely, particularly in the first days of combined therapy, and replace potassium and magnesium as required.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
