Summary

Talquetamab causes cytokine release syndrome, and elevated cytokines can temporarily suppress CYP450 enzymes. This may raise everolimus concentrations and the risk of toxicities such as stomatitis, infection and pneumonitis.

What this interaction means in plain words

Everolimus and Talquetamab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Everolimus belongs to Oncology; Talquetamab belongs to Oncology.

Evidence level: Low

Why it happens

Cytokine-mediated downregulation of CYP3A4 activity during cytokine release syndrome reduces everolimus metabolism.

What to do

Monitor for everolimus toxicity during talquetamab step-up dosing and for about two weeks after the first treatment dose, and during any CRS. Check everolimus trough levels and reduce the dose if concentrations are high.
