Summary

Fenfluramine is a serotonin-releasing agent used for seizures in Dravet and Lennox-Gastaut syndromes. Combined with tapentadol, which has serotonergic and noradrenergic activity, there is a theoretical risk of serotonin syndrome, and fenfluramine can itself cause somnolence.

What this interaction means in plain words

Fenfluramine and Tapentadol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fenfluramine belongs to Central Nervous System; Tapentadol belongs to Musculoskeletal.

Evidence level: Low

Why it happens

Additive serotonergic activity and possible additive CNS depression; the net effect is not well characterized.

What to do

Monitor for serotonin toxicity (agitation, tremor, clonus, hyperthermia) and for sedation. Use the lowest effective tapentadol dose.
