Summary

Losartan is converted to its more potent active metabolite E-3174 mainly by CYP2C9 (with CYP3A4). Fluconazole inhibits these enzymes, so conversion may fall and the antihypertensive effect may be reduced in some patients.

What this interaction means in plain words

Fluconazole and Losartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fluconazole belongs to Anti Fungals; Losartan belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Fluconazole inhibits CYP2C9 and CYP3A4, reducing conversion of losartan to E-3174.

What to do

Monitor blood pressure when fluconazole is started or stopped. Increase the losartan dose or switch to another ARB if control is lost.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
