Summary
Furosemide adds to the diuretic effect of hydrochlorothiazide and irbesartan, which can cause hypotension, volume depletion, electrolyte loss and acute kidney function decline, especially when starting an ARB in a patient on a loop diuretic. Furosemide-induced hypokalemia is partly offset by irbesartan, but hyponatremia and hypomagnesemia can still occur.
What this interaction means in plain words
Furosemide and Hydrochlorothiazide-irbesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Furosemide belongs to Cardiovascular; Hydrochlorothiazide-irbesartan belongs to Cardiovascular.
Why it happens
Sequential nephron blockade with loop and thiazide diuretics combined with RAAS inhibition produces additive natriuresis and reduced glomerular filtration pressure.
What to do
Monitor blood pressure, weight, serum sodium, potassium and creatinine after starting or adjusting therapy. Reduce the diuretic dose if volume depletion or hypotension develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
