For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Revefenacin's active metabolite is a substrate of OATP1B1 and OATP1B3, and gemfibrozil inhibits OATP1B1. Systemic exposure to the metabolite may rise, increasing anticholinergic effects such as dry mouth, urinary retention and tachycardia.

What this interaction means in plain words

Gemfibrozil and Revefenacin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Gemfibrozil belongs to Cardiovascular; Revefenacin belongs to Respiratory.

Evidence level: Moderate

Why it happens

Gemfibrozil inhibits OATP-mediated hepatic uptake of the active metabolite of revefenacin.

What to do

Labeling does not recommend coadministration with OATP1B1/1B3 inhibitors. Consider an alternative lipid-lowering agent or bronchodilator and monitor for anticholinergic effects.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
