Summary
Gemfibrozil strongly inhibits CYP2C8, a pathway contributing to tolebrutinib metabolism. Tolebrutinib exposure increases and formation of its M2 metabolite falls, which may raise adverse effects such as liver injury.
What this interaction means in plain words
Gemfibrozil and Tolebrutinib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Gemfibrozil belongs to Cardiovascular; Tolebrutinib belongs to Central Nervous System.
Why it happens
Inhibition of CYP2C8 (also by gemfibrozil glucuronide) reduces tolebrutinib clearance.
What to do
Avoid concomitant use and select another lipid-lowering agent such as a statin. If it cannot be avoided, monitor liver tests closely.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
