Summary
Both drugs can prolong the QTc interval, so using them together may add to the risk of ventricular arrhythmias including torsades de pointes. The risk is higher with electrolyte disturbance, bradycardia or underlying heart disease.
What this interaction means in plain words
Gemifloxacin and Trimipramine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Gemifloxacin belongs to Anti Bacterial; Trimipramine belongs to Central Nervous System.
Why it happens
Additive blockade of cardiac potassium channels (hERG) leading to delayed repolarisation.
What to do
Obtain a baseline ECG and correct hypokalemia and hypomagnesemia before combining. Consider an antibiotic without QT effects if the patient has other risk factors for QT prolongation.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
