Summary

Thiazide diuretics can cause volume depletion and electrolyte loss, and angiotensin receptor blockers reduce renal perfusion pressure in volume-depleted patients. Together with gentamicin, which is directly nephrotoxic and ototoxic, this raises the risk of acute kidney injury and aminoglycoside accumulation. Electrolyte disturbances such as hypokalemia or hypomagnesemia may also occur.

What this interaction means in plain words

Gentamicin and Hydrochlorothiazide-irbesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Gentamicin belongs to Anti Bacterial; Hydrochlorothiazide-irbesartan belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive renal hemodynamic compromise from diuretic-induced volume depletion and ARB efferent arteriolar dilation, combined with direct aminoglycoside tubular toxicity.

What to do

Monitor serum creatinine, potassium and magnesium regularly, and check gentamicin levels with dose adjustment for renal function. Consider temporarily withholding the diuretic/ARB combination during acute illness or volume depletion.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
