Summary
Aminoglycosides can cause renal magnesium and potassium wasting, and hypokalemia or hypomagnesemia increases the risk of QT prolongation and torsades de pointes with sotalol. Gentamicin may also impair renal function, which reduces sotalol clearance and raises its exposure.
What this interaction means in plain words
Gentamicin and Sotalol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Gentamicin belongs to Anti Bacterial; Sotalol belongs to Cardiovascular.
Why it happens
Gentamicin-induced electrolyte loss and possible renal impairment add to sotalol's QT-prolonging effect and reduce its renal elimination.
What to do
Monitor serum potassium, magnesium and renal function during gentamicin therapy and replace deficits promptly. Consider an ECG for QT interval in patients with additional risk factors.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

Frequently asked questions
Can I take Gentamicin with Sotalol?
MediHelp rates the interaction as moderate. Gentamicin and Sotalol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
How serious is the interaction between Gentamicin and Sotalol?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Monitor serum potassium, magnesium and renal function during gentamicin therapy and replace deficits promptly. Consider an ECG for QT interval in patients with additional risk factors.
