Summary
Combining a thiazide with a loop diuretic produces sequential nephron blockade and a stronger diuretic response. This increases the risk of hypokalemia, hypomagnesemia, hyponatremia and volume depletion.
What this interaction means in plain words
Hydrochlorothiazide and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide belongs to Cardiovascular; Torsemide belongs to Cardiovascular.
Why it happens
Additive diuresis and increased distal sodium delivery lead to enhanced urinary potassium and magnesium loss.
What to do
Monitor serum potassium, magnesium, sodium and renal function after starting or changing doses. Replace electrolytes as needed and reassess volume status.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
