Summary
Torsemide is a loop diuretic that adds to the diuretic and antihypertensive effect of this combination and promotes potassium and magnesium loss, partly offset by irbesartan. Excessive volume depletion, hypotension and renal function decline can occur, especially at initiation.
What this interaction means in plain words
Hydrochlorothiazide-irbesartan and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-irbesartan belongs to Cardiovascular; Torsemide belongs to Cardiovascular.
Why it happens
Additive diuresis and electrolyte wasting from sequential nephron blockade, combined with ARB-mediated reduction in glomerular filtration pressure.
What to do
Monitor blood pressure, serum potassium, magnesium, sodium and creatinine after initiation or dose changes. Reduce the diuretic dose if hypotension or volume depletion occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
