Summary

Hyoscyamine is a systemic antimuscarinic that can add to the anticholinergic effects of ipratropium, such as dry mouth, constipation, urinary retention and tachycardia. Inhaled ipratropium is poorly absorbed, so the additive effect is usually modest.

What this interaction means in plain words

Hyoscyamine and Ipratropium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Hyoscyamine belongs to Genitourinary; Ipratropium belongs to Respiratory.

Evidence level: Moderate

Why it happens

Additive muscarinic receptor blockade.

What to do

Monitor for urinary retention, constipation and heart rate changes, especially in older adults. Reduce the hyoscyamine dose if anticholinergic symptoms appear.
