Ibuprofen + Vasopressin

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

NSAIDs may potentiate the antidiuretic effect of vasopressin, increasing risk of water retention and hyponatremia; combined use may also increase risk of renal impairment, especially in hypovolemic or critically ill patients.

Mechanism of action

NSAIDs inhibit renal prostaglandin synthesis, removing a counter-regulatory brake on vasopressin’s V2-mediated water reabsorption in the collecting duct; both agents can reduce renal perfusion under some conditions.

Clinical management

Avoid NSAIDs during vasopressin therapy when possible; consider acetaminophen for analgesia/antipyresis. If concomitant use is necessary, monitor serum sodium and osmolality at baseline and at least daily, track fluid balance/urine output, and monitor renal function; discontinue or adjust therapy if hyponatremia or AKI develops.
