Summary

Icatibant blocks bradykinin B2 receptors, and bradykinin contributes to some of the effects of ACE inhibitors. Co-administration could theoretically reduce the antihypertensive effect of ramipril. Its use in ACE-inhibitor angioedema is not established as effective, and an interaction in practice is largely theoretical.

What this interaction means in plain words

Icatibant and Ramipril have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Icatibant belongs to Miscellaneous; Ramipril belongs to Cardiovascular.

Evidence level: Low

Why it happens

Pharmacodynamic antagonism through bradykinin B2 receptor blockade, opposing the bradykinin-mediated actions of ACE inhibition.

What to do

Monitor blood pressure during and after icatibant treatment and adjust antihypertensive therapy if control is lost.
