Summary

Icatibant blocks the bradykinin B2 receptor, and part of the blood pressure lowering effect of ACE inhibitors comes from raised bradykinin levels. Concurrent use may therefore reduce the antihypertensive effect of trandolapril. The clinical significance is uncertain and is probably small with short courses of icatibant.

What this interaction means in plain words

Icatibant and Trandolapril have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Icatibant belongs to Miscellaneous; Trandolapril belongs to Cardiovascular.

Evidence level: Low

Why it happens

Pharmacodynamic antagonism: icatibant blocks bradykinin B2 receptors, which contribute to the effects of ACE inhibition.

What to do

Monitor blood pressure when icatibant is given to patients on trandolapril. Icatibant is not recommended for ACE inhibitor-induced angioedema, so do not use it for that purpose.
