Summary

Infliximab (TNF blocker) and rilonacept (IL-1 blocker) each impair host defense, and combining cytokine inhibitors raises the risk of serious infection. Similar combinations of biologics have shown more infections without added benefit.

What this interaction means in plain words

Infliximab and Rilonacept can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Infliximab belongs to Musculoskeletal; Rilonacept belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Additive immunosuppression through simultaneous blockade of TNF-alpha and IL-1 signalling.

What to do

Avoid concurrent use where possible and choose a single biologic. If combined, monitor closely for signs of infection and interrupt therapy if a serious infection develops.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
