Summary

Itraconazole can raise the concentration of risperidone's active moiety and both drugs can prolong the QTc interval. This may cause more extrapyramidal symptoms, sedation, hypotension and arrhythmia risk.

What this interaction means in plain words

Itraconazole and Risperidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Itraconazole belongs to Anti Fungals; Risperidone belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

P-glycoprotein and CYP3A4 inhibition reduces risperidone clearance, plus additive QT effects.

What to do

Monitor for extrapyramidal and cardiovascular effects and consider reducing the risperidone dose. Obtain an ECG in patients with other QT risk factors.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
