Summary

Ribociclib is a moderate CYP3A4 inhibitor, and CYP3A4 is only one of several pathways involved in lacosamide clearance. Any rise in lacosamide exposure is expected to be small. Dose-related dizziness, PR interval prolongation and sedation could be slightly more likely.

What this interaction means in plain words

Lacosamide and Ribociclib have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Lacosamide belongs to Central Nervous System; Ribociclib belongs to Oncology.

Evidence level: Low

Why it happens

Ribociclib weakly inhibits CYP3A4, one of the minor metabolic routes of lacosamide (alongside CYP2C9 and CYP2C19).

What to do

No dose change is normally needed. Watch for dizziness, ataxia and conduction effects, and consider an ECG if the patient has pre-existing cardiac conduction disease, since ribociclib also prolongs the QT interval.

What to ask your pharmacist
