Summary
Lonafarnib is a strong CYP3A inhibitor and may considerably increase ranolazine exposure. Ranolazine is a weak CYP3A inhibitor and may in turn raise lonafarnib levels, with the added risk of QT prolongation and toxicity.
What this interaction means in plain words
Lonafarnib and Ranolazine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Lonafarnib belongs to Miscellaneous; Ranolazine belongs to Cardiovascular.
Why it happens
Lonafarnib inhibits CYP3A4 metabolism of ranolazine, and ranolazine weakly inhibits CYP3A4 metabolism of lonafarnib.
What to do
Avoid the combination, as strong CYP3A inhibitors are contraindicated with ranolazine. Use an alternative antianginal agent.
