Summary

Lopinavir is co-formulated with ritonavir, a strong CYP3A4 inhibitor, and is expected to raise lurasidone exposure considerably. This increases the risk of sedation, extrapyramidal symptoms and other dose-related toxicity. The combination is contraindicated.

What this interaction means in plain words

Lopinavir and Lurasidone are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Lopinavir belongs to Anti Virals; Lurasidone belongs to Central Nervous System.

Evidence level: High

Why it happens

Ritonavir-boosted lopinavir inhibits CYP3A4-mediated metabolism of lurasidone.

What to do

Do not coadminister; choose an antipsychotic less dependent on CYP3A4 or a different antiretroviral.

What to ask your pharmacist
