Summary
Prolonged NSAID use can blunt the blood pressure lowering effect of beta-blockers such as sotalol. NSAIDs can also reduce renal function and cause fluid retention, which may raise sotalol exposure since it is renally cleared and increase proarrhythmic risk.
What this interaction means in plain words
Lornoxicam and Sotalol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Lornoxicam belongs to Musculoskeletal; Sotalol belongs to Cardiovascular.
Why it happens
NSAID inhibition of prostaglandin synthesis causes sodium and water retention and reduced renal perfusion, opposing the antihypertensive effect and potentially reducing sotalol clearance.
What to do
Monitor blood pressure and renal function when lornoxicam is used for more than a few days with sotalol. Use the lowest effective dose for the shortest time, or consider paracetamol.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
