Summary
Metoclopramide with an antipsychotic such as quetiapine adds dopamine blockade and raises the risk of extrapyramidal effects, tardive dyskinesia and neuroleptic malignant syndrome. Quetiapine has relatively weak D2 binding, so the risk may be lower than with other antipsychotics, but it is not eliminated.
What this interaction means in plain words
Metoclopramide and Quetiapine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Metoclopramide belongs to Gastrointestinal Tract; Quetiapine belongs to Central Nervous System.
Why it happens
Additive dopamine D2 receptor antagonism.
What to do
Avoid the combination where possible and use an alternative antiemetic. If it cannot be avoided, use the shortest duration and monitor for movement disorders and rigidity or fever.
