Summary
Tramadol is activated by CYP2D6 to O-desmethyltramadol, which provides much of its opioid analgesia. Mirabegron inhibition of CYP2D6 may reduce analgesia while raising parent tramadol levels, which contribute to serotonergic and seizure risk.
What this interaction means in plain words
Mirabegron and Tramadol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Mirabegron belongs to Genitourinary; Tramadol belongs to Musculoskeletal.
Why it happens
Mirabegron inhibits CYP2D6, reducing conversion of tramadol to its active opioid metabolite.
What to do
Monitor pain control and for serotonergic or neurological adverse effects. Consider a non-CYP2D6-dependent analgesic if the response is inadequate.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
