Summary
Mirtazapine is sedating and increases serotonergic and noradrenergic transmission, so with an opioid it can add to drowsiness and respiratory depression. Serotonin syndrome is possible but less likely with pentazocine than with strongly serotonergic opioids.
What this interaction means in plain words
Mirtazapine and Pentazocine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Mirtazapine belongs to Central Nervous System; Pentazocine belongs to Musculoskeletal.
Why it happens
Additive CNS depression from antihistaminic effects and opioid activity, with possible additive serotonergic effects.
What to do
Monitor for sedation and respiratory depression, and educate patients on features of serotonin syndrome. Lower doses if drowsiness is excessive.
