Summary
Vortioxetine, like other serotonergic antidepressants, impairs platelet serotonin uptake and can increase bleeding tendency. Combined with piroxicam, which inhibits platelet function and irritates the gastric mucosa, the risk of gastrointestinal and other bleeding is increased.
What this interaction means in plain words
Piroxicam and Vortioxetine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Piroxicam belongs to Musculoskeletal; Vortioxetine belongs to Central Nervous System.
Why it happens
Additive impairment of platelet aggregation (SSRI-type serotonin depletion plus NSAID COX-1 inhibition) with NSAID mucosal injury.
What to do
Monitor for signs of GI bleeding and bruising, and consider gastroprotection with a proton pump inhibitor in higher-risk patients. Use the lowest effective NSAID dose for the shortest time.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
